Hypnotherapy Agreement

HYPNOTHERAPY AGREEMENT – Heid Hypnotherapy

Between the client (named via electronic signature) and the hypnotherapist:

This Agreement sets out the terms and conditions relating to our work together.

1.     The hypnotherapist is a member of the General Hypnotherapy Register and therefore abides by their code of conduct which includes regularly continuing their learning and development.

2. Heid Hypnotherapy is a UK based company working under local (UK) legislation, registration, and industry best practise. The client agrees when seeking a hypnotherapy service from the hypnotherapist, that they are governed by these standards rather than their own local laws during this interaction.

2.     The hypnotherapist and client agree to work together to address the issues presented by the client, however due to the nature of the treatment, no guarantee of a cure can be given. However, the hypnotherapist agrees to ensuring they give their best effort and skills to work towards the goals agreed.

3.     The cost of each session may differ depending on the service required. All prices are clearly displayed on the web booking appointment system and will be charged at the point of booking.

4.     Fee is for therapist’s appointment slot which is usually within one-hour sessions. The client reserves this time but can choose not to use the entire hour if they arrive late or if they wish to finish early.

5.     Clients can change the date of sessions using the web booking scheduling system with up to 24 hours’ notice. They can do this as many times as they need to.

6.     Cancellations with less than 24 hours’ notice will result in the client being liable for the cost of the missed session.

7.     Refunds for cancellations with more than 24 hours’ notice but without the client rebooking will be down to the hypnotherapist’s discretion and are not guaranteed.

8. Multi session deals are for the use of the person making and attending the original booking. They cannot be exchanged or adapted. The additional sessions can be exchanged for any hypnotherapy service including Relaxation, Past Lives, or Spiritually focused work.

9.     Contact between sessions will be limited to telephone or email during the hypnotherapist’s working hours.

10.     For optimum success the client agrees to make reasonable use of therapeutic suggestions within sessions including listening to voice recordings and use reasonable efforts to adopt agreed actions (for example behavioural or wellness suggestions where appropriate)

11.  The client agrees to take responsibility for their choices, actions, and life’s outcomes.

12.  Hypnotherapy voice recordings, videos, mP3s should never be listened to whilst driving or operating heavy machinery.

13.  Materials given to the client by the hypnotherapist are subject to copyright laws and must not be shared or reproduced for any commercial purpose or financial gain.

14.  Antisocial behaviour by the client will result in immediate cessation of treatment.

15.  The hypnotherapist will ensure confidentiality is maintained unless there are exceptional circumstances. This includes a safeguarding risk posing a serious threat to the client or others.

16.  The hypnotherapist will take part in regular supervision which may include the discussion of individual sessions. This will be within a confidential environment and will not include identifying information about the client therefore does not breach professional confidentiality.

17.  Information collected during sessions will be protected under GDPR unless there is a request to disclose under Court Order (civil, criminal, or coroner’s Court) or where not to disclose would cause a risk of serious harm.

18.  The hypnotherapist will only share information with an NHS medical practitioner or other health professional with the written agreement of the client.

19.  The client is responsible for seeking the advice of a qualified medical practitioner before commencing treatment if they have questions over their own suitability and safety for hypnotherapy treatment.

20.  The client is responsible for declaring any reason/s why hypnotherapy may present a risk to their physical or mental health. Information left out of initial forms is at the client’s own risk.

21.  During the process of treatment there may be reason to review unpleasant memories or experience uncomfortable emotions. This may result in confusion or setbacks. The client acknowledges and accepts these potential risks.

22.  The client will seek the advice of a qualified medical practitioner before stopping or altering any medication or treatment they are currently receiving. The hypnotherapist will never suggest the client goes against the advice of health professionals the client is currently under the care of.

23.  The hypnotherapist has the right to terminate the therapeutic relationship and refer the client to other specialists if they consider the client’s needs are outside of their expertise.

24.  The hypnotherapist will ensure the client has been given the link to this agreement via email prior to commencing their first appointment. By booking and attending the session, the client consents to working within the framework of this agreement.
